3. Fax or mail your form along with copies of your prescriptions.
4. Receive medications via the mail.

Note: We require valid prescriptions written by a U.S. doctor. Once an order has been placed we require that the prescriptions be faxed or mailed to us along with a copy of the patient's photo identification (driver’s license, birth certificate or passport).
